1、需要3名相關(guān)醫(yī)學(xué)專業(yè)畢業(yè)的大專以上(含大專)文憑產(chǎn)品質(zhì)量監(jiān)督檢測(cè)人員(其中一個(gè)為質(zhì)量檢測(cè)負(fù)責(zé)人);

2、三名質(zhì)量監(jiān)督檢測(cè)人員的身份證和畢業(yè)證復(fù)印件、工作簡(jiǎn)歷;(食品藥監(jiān)局老師過(guò)來(lái)場(chǎng)地核查約談的時(shí)候需要提供相關(guān)檢測(cè)人員的身份證和畢業(yè)證原件，并本人到場(chǎng));
